Payment Terms: Services will be invoiced upon completion of the work. Please complete the accompanying payment form below to prevent delays in scheduling your service request. I agree to pay CodeBlue Technology for services performed in the amounts specified within this rate agreement.
CodeBlue Technology will invoice the Client for the services rendered at the rates specified above. All Professional Services will be invoiced upon completion of the work and due within thirty (30) days of mailing. Past due bills may be subject to a late fee of one and one-half percent (1.5%) per month. If any invoice goes past due and is sent to a collections agency or attorney, the Client shall be responsible for paying a collection fee of thirty percent (30%) on any outstanding balance. Code Blue reserves the right to suspend services if any invoice owed to Code Blue Technology is past due thirty (30) days or more from the date of invoice.
